Goldenseal has been used for GI disturbances and as an anti-infective and is often used for colds, sinus and upper respiratory infections, sometimes in combination with the herb echinacea. Goldenseal is used in conjunction with standard antimicrobial therapy. For travelers diarrhea, use prophylactically 1 week before, during and 1 week after. Do not use for greater than 2 months at a time. Pharmacology: The primary constituents of Goldenseal are berberine and hydrastine. Berberine has antimicrobial activity against a broad array of pathogens, including viruses, fungi, parasites, and bacteria and Berberine has also been reported to have antipyretic effects. Hydrastine has been reported to have antitussive, antiperistaltic and antihypertensive effects. Berberine compounds cause uterine stimulation, and should be avoided in pregnancy. The alkaloids in Goldenseal have astringent activity which appears to contribute its anti-inflammatory activity on mucous membranes.High doses may cause hypotension, and hypoglycemic effects have been observed.

Pharmacology:

Astragalus contains isoflavones, triterpenoids, and saponins, including astragalosides I-VIII. Polysaccharides contained in astragalus have been shown to stimulate natural killer (NK) cells, augment T-cell function, and increase interferon production. Astragalus administration has been shown to increase phagocytosis by reticuloendothelial cells, decrease T-suppressor cell function, and improve T-killer cell function. Triterpenoids and saponins have a structural similarity to steroid hormone precursors and appear to increase adrenal activity. Astragalus may decrease cyclophosphamide-induced immune suppression.

Theoretical Cautions and Contraindications:

Use of Goldenseal is contraindicated in pregnancy and lactation and in individuals with cardiovascular disease since high doses may cause hypotension, bradycardia or gastrointestinal distress. Berberine and hydrastine alkaloids are potentially toxic-excessive use should be avoided. Overdosing has been reported to induce CNS depression. High-dose hydrastine has been associated with hypertension, hyper-reflexia, and seizures. Extended use of high doses has been associated in some reports with neuroexcitation, hallucinations, delirium, and gastrointestinal disorders. Overdose has been associated with myocardial damage and respiratory failure. Berberine has been shown to decrease tetracycline absorption. Use as a douche is contraindicated because of its potential local ulcerative effects. Because of hypoglycemic effect, monitor for diabetes.

Theoretical Interactions:

Avoid concomitant use of anticoagulants, antihypertensive agents, Beta blockers, calcium channel blockers, digoxin, tetracyclines, and CNS depressants.
